From x-rays to AI: a biomedical engineer’s reflections on 50 years of progress

In 1987, I began my journey as a radiotherapy engineer – armed with tools, determination and a passion for supporting cancer care. I subsequently moved across to diagnostic imaging and nearly four decades later the landscape of radiography and radiotherapy has changed beyond recognition.

Back then, treatments were far less precise and imaging technology was in its infancy compared to today’s standards. Linear accelerators were mechanical marvels, but nothing like the sleek, software-integrated machines we see now. In imaging, the arrival of spectral CT, accelerated MRI reconstruction and collaborative imaging using AI has transformed outcomes and patient experience alike.

This evolution hasn’t just benefited patients, it has redefined what it means to work in this field. Departments have grown into more positive, hopeful spaces. Patients are living longer, with a better quality of life, and that optimism filters through every corridor. For staff, including those of us in engineering and technical support, it is more rewarding than ever to be part of that journey.

Yes, we face challenges – staff shortages, financial constraints and the relentless pace of technological change – but the opportunities are just as abundant. AI is beginning to shoulder routine tasks, freeing up clinicians and engineers to focus on innovation and care. The momentum is exhilarating.

If I had the chance to go back, I wouldn’t change a thing. This career has given me purpose, satisfaction and a community of colleagues and friends who inspire me every day. Radiotherapy and radiography may have changed, but the privilege of being part of it remains constant.
